BY USING OUR SERVICES, YOU AGREE TO COMPLY WITH THE FOLLOWING TERMS AND CONDITIONS:
Appointments: To schedule an appointment with Molldrem Family Dentistry, you can call our office or book online. We require at least 24 hours notice for any appointment changes or cancellations. Failure to provide adequate notice may result in a cancellation fee.
Payment: We accept cash, checks, and credit cards as payment for our services. We also accept most dental insurance plans. Payment is due at the time of service unless prior arrangements have been made with our office. We reserve the right to charge interest on any past-due balances.
Whitening for Life Program: Our Whitening for Life program provides safe and effective whitening products to patients so long as they get regular cleaning and checkups every 6 months. Missing regular hygiene appointments will result in the termination of current whitening-for-life refills, patients will need to pay $99 to renew the program. Loss of trays by the patient will incur a $199 fee to receive a replacement. Terms and conditions apply at the time of the program.
Non-Discrimination Policy: Molldrem Family Dentistry does not discriminate on the basis of race, ethnicity, religion, gender, sexual orientation, age, or disability in providing our services.
Confidentiality: We take patient confidentiality very seriously and comply with all HIPAA regulations. We will not disclose any patient information without the patient’s express written consent.
Limitation of Liability: Molldrem Family Dentistry is not responsible for any damages or injuries that occur as a result of the use of our services or equipment. We are not liable for any damages or injuries that occur due to the patient’s failure to disclose relevant medical information.
Dispute Resolution: Any disputes between the patient and Molldrem Family Dentistry will be resolved through arbitration in accordance with the rules of the American Arbitration Association.
Modification of Terms: We reserve the right to modify these terms and conditions at any time. Any changes will be posted on our website and will take effect immediately upon posting.
